Financial InformationThe Center for Reproductive Medicine consists of two separate entities:The Center for Reproductive Medicine, P.A. (CRM)CRM participates with most major insurance carriers and provides services for physician consultation, infertility diagnostic testing, monitoring of infertility treatment such as ultrasound, laboratory testing and intrauterine insemination. Insurance often covers services provided by the Center for Reproductive Medicine, depending on your insurance network provisions. It is important to remember that if your insurance plan does not cover infertility treatment such as intrauterine insemination or in vitro fertilization, then it will not pay benefits to any provider, participating or non-participating, in-network or out-of-network. Advanced Reproductive Technologies, P.A. (ART)ART provides In Vitro Fertilization services. Advanced Reproductive Technologies does not accept insurance reimbursement for any services provided. Prepayment for In Vitro Services is required in advance of the IVF cycle. Since Advanced Reproductive Technologies does not participate with insurance providers, insurance forms and instructions will be provided for submission to your insurance carrier. If you have insurance coverage for IVF and if you have out-of-network benefits for IVF, your insurance carrier will send reimbursement payments directly to you.
